Useful Forms

The school day

The classroom doors open at 8.50am.

Registration and offical start of the school day - 9.00am

KS1 morning break - 10.30am - 10.45am / KS2 morning break 10.45am / 11.00am

Lunch break for all year groups - 12.00pm - 1.00pm

End of school day. Pick up from classroom doors - 3.20pm

Pick up can continue until 3.30pm when classroom doors will be closed.

Medication Consent Form:

If for some reason your child requires medication to be administered during the school day, you must complete the Medication Consent Form and bring it to the school office with the childs medication clearly labelled.

Inhaler consent form:

If your child requires an inhaler in school, you must provide the inhaler alongside a consent form and the medical form that is supplied with the inhaler. Please note that it is the parent and carers responsibility to ensure that their childs inhaler is in-date and in school with them every day. Inhalers are kept in boxes within each classroom and are also taken out at playtimes and during PE lessons. This is to ensure that inhalers are with the child at all times.

Count Me In Form:

All children who are at school; and were under 19 at the start of the academic year; and are claiming or have parents who are claiming:

• Income support; or • Income Based Job Seekers Allowance; or • Income Related Employment and Support Allowance or • Child Tax Credit, as long as you do not get Working Tax Credit and whose annual income (as assessed by HM Revenue & Customs) does not exceed £16,190; or • Support under part VI of the Immigration & asylum Act 1999; or • Guarantee element of State Pension Credit • Universal Credit (with an annual take home pay of no more than £7,400, as assessed by earnings from up to three of your most recent assessment periods)

Are entitled to apply for Free School Meals and for the school to receive additional funding (Pupil Premium) to help support children at school.

Free or subsidised School CoolMilk Form:

Children under the age of 5 years old are entitled to free school milk each day. Children 5 years and over are offered subsidised milk costing approximatly £14 a term.
